I, hereby, apply for membership in the San Joaquin Valley Swiss Club as requested below:

REGULAR MEMBERSHIP: Any Swiss or American of Swiss Descent

SPOUSAL MEMBER: Any Spouse of a Regular or Social Member. Co-application for Spousal Membership. No extra dues.

Application is for Spousal Membership only. My spouse (legal husband or wife) is already a member in good standing,

therefore no fee or dues are required.

PASSIVE MEMBERSHIP: Any non-Swiss whose spouse is Swiss or American-Swiss

Automatically becomes a Regular Member after 5yrs

SOCIAL MEMBER: Any non-Swiss. Automatically becomes a Regular Member after 5 yrs

Enclosed are my first calendar year’s dues of $40.00, plus the one time initiation fee of $10.00 making a total of $50.00, which I understand is

refundable if for any reason this application is not accepted (Make check payable to SJVSC).

I understand that I will be contacted to volunteer and as a member need to help at least once a year.
